Release Of Liability

Informed Consent and Acknowledgment

I, the undersigned, understand that participation in the youth basketball club activities, including tournaments, competitive games, team practices, training sessions,  seasonal camps, and after-school programs, involves physical activities that carry certain risks. I am aware of the risks and hazards associated with these activities, which include but are not limited to, physical injury, potential exposure to COVID-19, and other unforeseen accidents.

Release of Liability

In consideration for the opportunity to participate, I hereby release and hold harmless The Portland Reign, its coaches, staff, volunteers, and affiliates, from any and all liability, claims, or actions arising from injuries, accidents, or other incidents that may occur while my child is participating in club activities, whether caused by negligence or otherwise.

Medical Consent

In the event of a medical emergency, I authorize The Portland Reign staff and volunteers to seek emergency medical care as deemed necessary for the health and safety of my child. I understand that I am financially responsible for any medical treatment incurred.

Photo and Media Release

I give permission for my child to be photographed or recorded during club activities for promotional use by The Portland Reign.

Acknowledgment of Understanding

I have read this waiver and release of liability and fully understand its terms. I confirm that I am signing freely and voluntarily, intending to grant a complete and unconditional release of all liability.
